Parents Guardians Release and Permissions

I, ______________________________, as the legal guardian of above named child(ren), release, absolve and hold harmless NJ Onnuri Community Church. Its teachers, volnteers, staff and directors in case of any incidence that may occur in relation to NJ Onnuri Summer Camp.

I give my child permission to attend NJ Onnuri Summer Camp and participate in all related activities. I hereby consent to the talking of photographs or videos of the registered child. I give permissions to use such photos and videos for non-profit purposs. I have read the release and approve of its terms.
