Thank you for choosing to book your FUN vacation with me and taking the time to answer the following questions!  I can use this information to create a more accurate quote and better envision your dream Carnival Cruise vacation.  I can't wait to get started planning with you!  :)

Would you like to include travel insurance/protection?  The cost starts at $49+ per person varying by sailing, and the cost will be provided to you prior to adding it.  This must be paid IN FULL when added to your vacation package.  *The amount paid for travel protection is NON-REFUNDABLE under any circumstances.   *

Any CELEBRATIONS during your cruise?  (First Cruise, birthday, anniversary, graduation, etc.)  Is this a SURPRISE trip?
Does anyone in your travel party have food allergies/dietary restrictions or special needs that I need to make Carnival aware of?  
  Do any guests require use of a CPAP machine?
