Parent Group Registration Form
This registration form is for anyone NEW to our groups. If you have attended a parent group in the past you should be getting emails with the link to join that group each month. Please email your group's facilitator or connected@familyvoicesmn.org if you have questions or are not receiving these monthly emails. Virtual meetings are open to anyone in MN who is raising a child with special health needs / disabilities. Some groups are specific to age groups and the Dads group is for people who are in dad roles. After you complete this form, expect to hear from one of our Parent Support Navigators by email and/or phone. We will set up a time to talk with you by phone. During this phone call we will offer any information and support that might be relevant and help you find a parent group that suits your schedule and needs.
