I understand that I will be receiving therapy services from a student intern who is under the supervision of Center for Wellness & Clinical Development and the Field Placement Office of their educational institution. All interns are supervised by Dr. Brittany Peters, LCSW, MCAP, QS and the acting supervisor for their educational institution. Student interns are bound by the ethical guidelines of their profession and adhere to the guidelines specified by HIPPA, Telehealth Service Consent, and Internship Supervision Agreement of their educational institution. Clients may terminate this agreement at any time, but termination of this agreement will require transfer to another provider or outside organization.
