Assalamualaikum dear brothers and sisters, In OMA, we update our membership list every semester. This form is for the Fall Term membership update. Please fill it carefully. ⚠️ If you are only visiting and using the prayer room as a guest (once or twice), you do not need to fill this form. As part of our commitment with Osaka Ibaraki Mosque (OIM), all registered OMA members are also registered as members of OIM. That means OMA members do not need to fill the OIM membership form separately. We will share with OIM only your name.
