Incident/Complaint Form
Incident/Complaint Form
This form is to be used to file complaints that have occurred with fellow E-Board members or within the Latinx Business Association. The purpose of the form is for documentation and accountability purposes. This will help with addressing matters and take the appropriate professional measures. Any and all incidents are important and we highly encourage when submitting a complaint that information is accurate and to keep in mind to separate the Personal from the Professional. We also highly encourage that any violations of the Constitution be sent in. Depending on the circumstances and incidents, mediation meetings may be scheduled if they persist with the same E-board members. Statements and all information included in this form will be sent to the current HR Director. Permission will be asked if any information is shared with other Executive board members.
