1) New Clients: To ensure a smooth first visit, please complete the following forms before your appointment: (a) Client Intake Form (b) Liability Waiver and Consent Form 2) Retinoid Use: Please discontinue the use of prescription-grade retinoids 7 days prior to your appointment. 3) Health Considerations: If you’re feeling unwell or sick, Erika kindly asks that you reschedule your appointment. 4) Parking Information: - Street Parking - 18th Ave and Geary Blvd Lot - Geary Mall Garage *For convenience, it’s recommended to extend metered time by at least 20 minutes beyond the estimated end of your service. 5) Arrival Instructions: Upon arrival, please enter the building using the door code provided in your confirmation email. Then, proceed to Suite 200 (located to your left if walking up, or right if using the elevator). Erika will be waiting for you in the reception area at your scheduled appointment time.
