Step 3: print and complete athletic checklist and physical form to take to your appointment

Step 4: complete self medication form (if it applies to you)

Step 5: Complete Booster Membership Form

FINAL STEP: Complete EduCare form for 2024-2025 (REQUIRED EVERY SCHOOL YEAR)
