PURPOSE OF REFERRAL
Psychological Assessment for Hyperactivity & Developmental Functioning

BRIEF CLINICAL HISTORY


The child is 1st of Twin; born out of non-consanguineous parents; FTND-LSCS with
immediate birth cry and birth weight of 2.3 kgs. No significant prenatal, perinatal or
postnatal complications were reported. No delays in developmental milestones
reported, except started walking at the age of 17 months. Father reported that he
cant sit at one place; and his understanding is poor in terms of he cant draw a
line or write alphabets; confused with terms like aage/peeche; cant do cycling
properly; cant close his shirt-buttons.
