Voluntary Payroll Deduction Authorization

The employee authorizes their employer to make a voluntary payroll deduction for a specified reason over a set period of time. The deduction will be for a set amount in Philippine Pesos (PhP) beginning on a start date and ending on an end date, or until the total amount has been deducted. The employee acknowledges authorizing the deduction in exchange for a good or service received and indicates whether the remaining balance can or cannot be deducted from final wages if employment ends before the full deduction is made.
