Mastering DepEd School Forms SF1-10

This 3 hour session aims to teach teachers and administrative assistants how to properly fill out and maintain various school forms. Key Understanding/Input  The different school forms are: SF1 (School Register); SF2 (Daily Attendance
Report of Learners); SF3 (Books issued and Returned); SF4 (Monthly
Learner’s Movement and Attendance); SF 5 (Report on Promotion & Level
of Proficiency); SF6 (Summarized Report of Promotion & Level of
Proficiency); SF7 (School Personnel Basic Profile and Assignment); SF8
(Learner’s basic Health and Nutrition Report); SF9 (Learner’s Report Card);
SF10 (
 SF1, SF2, SF3, SF4, SF5, SF8, SF9 and SF10 are to be prepared by the class
adviser while SF6 and SF7 are to be prepared by the ADAS.
