INSTRUCTIONS
The candidate is directed to read the following instructions carefully before filling up the admission form:-
1. Candidate (male/female) must attach recent scanned photograph (with white/sky blue background).
2. Incomplete or invalid admission form is not accepted at all.
3. Admission forms will be received according to the dates notified by the University. Submit the admission form to the office
after pasting the original fee challan within stipulated dates otherwise admission form will be rejected.
4. Failed/Exempted candidates may submit their admission forms with requisite fee Challan to the University Office within 15
days after declaration of the Result.
5. Deposit the requisite fee in any online branch of Habib Bank Limited and United Bank Limited. In case of any deficiency in
fee, admission form will not be processed.
6. Money orders, postal orders and cheques are not acceptable.
7. The examination will be held on the given date/dates as per date sheet.
8. Roll No. Slip will be issued provisionally. Result will be quashed if found ineligible or if found involved in any illegal activity.
9. Un-authorized change of Centre/Subject/Group is strictly prohibited. No mark of identification should be made on the
answer book. Failing to which UMC and FIR will be registered.
10. Do not make any appeal to the examiner. Don’t use obscene language in the answer book and don’t misbehave with the
supervisory staff.
11. Sitting arrangement of the candidates will be according to the list provided to the supervisory staff. In case a candidate is
found copying or facilitating other/s, both will be expelled from the examination centre.
12. No candidate can leave his seat or centre without permission of Superintendent.
13. Any candidate who stages a walk out/boycott or instigates others to do so in the Examination Hall or around any
Examination Hall shall be liable for all the damages.
14. The following copies must be attached with the admission form otherwise the admission form will not be accepted.

Copy of Matriculation, Intermediate, Bachelor Certificates / Result Cards
Photocopy of National ID Card (both of candidate’s and father’s/guardian’s)
15. Errors and Omissions accepted (E&OE) and University of the Punjab is not liable to respond any legal or court
proceedings.

18. I hereby declare that all the particulars are correct and that in case of any difficulty arising out of inaccuracy therein, I shall
be responsible for the consequences, I abide by the Statutes, regulations and rules etc. framed by the University from time
to time and shall be liable to any penalty including rustication/expulsion, in case of violation on my part.
