I. OVERVIEW

PHO Medical Team and Vaccination Team arrived at [Link], Tubigon at 8:44am,
immediately set up the vaccination area and started vaccination to serve the residents of
Tubigon. Dr. Estrella P. Lim immediately started consultations with the patients waiting, Medical
MedTech Mark Miñoza processed 29 urinalysis, 35 CBS, Medical Nurse Cristine Sajonia
conducted Covid19 vaccine information dissemination and explained misinformation about the
Covid19 vaccines, promoting and encouraging people to be vaccinated. Dr. Emerson Mateo MD,
explained facts, information about various cancers related to women's health. The rest of the
medical team, Nurse Dan Blyke D. Bantuga, Nurse Eloise Therese C. Saldaña Encoders Jim E.
Oppus, Lelanie V. Tejano provides screening, counseling, vaccination services, administering
Pfizer Vaccine and post monitoring to 12 patients. Left the scene at 3pm

III. RECOMMENDATIONS

1. Continue to cooperate with LGUs and other agencies in medical missions designed to
better the plight of the Bol-anons.
2. Coordinate with the other physicians that we need to make sure that the program has
already concluded before leaving the venue.
3. Communicate with the event organizers about the difficulties experienced in the field to
prevent it from re-occuring during future events.
