1 FotoFinder dermoscope dynamic

FotoFinder dermoscope dynamic provides an easy-to-use interface
which is adapted to the demands of the modern dermatological
practice. It allows documenting and analyzing different skin and hair
conditions.
Main fields of application:
• Long-term observation and documentation of melanocytic nevi using
digital dermoscopy
• Quick skin cancer screening, more comfortable and efficient than
with a hand dermatoscope.
• Analysis of nevi and clinical images with expert systems Tuebinger
Moleanalyze1 and Bodyscan pro to find changes.
• Fluorescence diagnosis to delineate cutaneous precancerous stages
and tumors (such as Actinic Keratosis or Basal Cell Carcinoma) from
the healthy skin.
• Digital trichogram with Trichoscan.
For standardized full body and “before and after” photography the
FotoFinder modules bodystudio and mediscope are available.

1.1 Special software features

Marking lesions
Suspicious moles can easily be marked. These marked lesions can
then be examined through digital dermoscopy using the special
video camera medicam. All microscopic images are directly linked to
the markers and stored in the patient data base.
CamControl
FotoFinder CamControl provides a direct connection between
camera and computer and allows FotoFinder dermoscope dynamic
software to control all camera settings. Per click, all images are
directly stored within the database and entered in the patient record.
For monitoring, the same zoom etc. will be selected by the software.
1
Note: Moleanalyzer is known as Dynamole in the USA.

1
FotoFinder dermoscope dynamic manual

SmartGallery
The intelligent picture gallery sorts overview and microscopic images
and helps you to find the right pictures for monitoring your patients.
SmartZoom
The zoom feature allows you to simultaneously zoom in or out in two
pictures. That’s how you will always see the right detail.
Xtrascale
The integrated measure module helps you to quantify lesions by
calculating lengths, angles and areas.
QueryGenerator
The flexible search engine allows queries for all combinations of
criteria which are available in the database; e.g. search for all last
year’s cases of melanoma in women older than 50 years.
Image Import/Export
Import and export patient records in the special FotoFinder exchange
format (fxf) to ensure maximum security. Single images can also be
exported in standard format to be used for papers, presentations etc.
Reports
Print images in predefined layouts directly from within the software.
You will obtain brilliant print outs of overview and microscopic
images to hand them over to your patients.
BackupGuard
Every time the program is started, the BackupGuard checks the
backup data and the backup settings for errors and notifies you if
your data is in danger.
Safety Instructionss and notifies you if your data is in danger.

2 Safety Instructions

2.1 Basic Notes

• Before using FotoFinder it is absolutely necessary to read the
complete manual.
• The given security instructions must be followed by the user!
• Before cleaning the camera it is important to disconnect the main
power supply.
• As far as possible, the system shouldn’t be turned on if…
… the power supply line is visibly damaged.
… the camera has fallen down.
• Any changes in the system that you intend to do must be reported to
the manufacturer. Otherwise the warranty is void!
• The system may not suffer exposure to extensive humidity.
• The manufacturer will not assume liability for any damages if the
system is not used properly or the system configuration has been
changed.
• During the system set-up please make sure that there is sufficient air
supply in order to avoid an accumulation of heat.
• The system may not be placed near heat sources like radiators or on
places where it is exposed to direct solar radiation, unusually high
dust, mechanical vibrations or shocks.
• The system may not be placed near places which generate a strong
magnetic field such as power lines or converters.
• If any malfunction appears, please contact the manufacturer or your
distributor immediately.

3
FotoFinder dermoscope dynamic manual

2.2 Safety Controls

Every time before using the system please check if there are any
damages. It is especially important to check if the power supply lines
or the lenses are damaged.
Please check the system regularly according to the generally
accepted technical rules, at least every 12 months.

2.3 Cleaning and Disinfection

Please clean the cases, control panels and control elements with a soft
cloth moistened slightly with a mild detergent. Do not use abrasives,
scrubbing brushes or dissolvers like alcohol or benzenes.
Clean the display with a soft cloth. If you use a glass detergent, please
make sure that it doesn’t contain antistatic fluids or similar additives
because they might cause scratches on the screen’s coating.
Dust on the lenses can be wiped off with a brush or a blower.
If you use the optional lenses, please pay attention to the following
instructions:
• The glass tube of the microscope lens must be disinfected before
every use. If you’re using Kodan immersion liquid, the glass tube will
be disinfected automatically during use. However, this is not the
case if dermoscopy oil is being used. For that reason we
recommend utilizing Kodan.
• If you use the optional polarization kit and work without immersion
liquid, make sure to disinfect the top piece regularly.
• For cleaning take off the glass tube. Use a high-quality glass cleaner
and a non-fuzzing cloth to clean the tube from inside and outside.
Then polish the tube with a dry cloth. To clean both sides of the
microscopy lens moisten the cloth with only some glass cleaner.
From time to time lubricate the rubber ring that holds the glass tube
with some petroleum jelly, but avoid bringing it on the lens. When
sticking the tube into the microscope lens again please make sure
that you stick it in completely! Otherwise the camera’s integrated
auto focus won’t work correctly when taking micro images.

2.4 Handling the medicam

FotoFinder medicam is an electronic software-controlled video-
dermoscope, developed to take microscopic and macroscopic
pictures of the patient’s intact skin surface, especially of moles, in
less than 60 minutes.
Some general instructions before using the medicam:
• Do not deconstruct the camera.
• Do not twist the camera cable or step on it.
• Do not spill any liquids on the camera.
• Do not touch the lens of the camera.
• Be careful when taking off the microscope lens and putting it
on a glass table or similar surfaces because it may easily be
scratched.
• Before cleaning the camera, please disconnect the complete
system.
• If the camera makes any unusual noise, emits smoke or un-
usual odor, please disconnect the whole system immediately
and contact your distributor or FotoFinder Systems GmbH.
• When fixing the glass tube on the microscope lens, make
sure to stick it in correctly. Wrong handling may prevent the
integrated automatic focus of the camera from working
correctly.
• We strongly recommend using alcohol spray (e.g. Kodan).
When using immersion oil the inside of the tube might get
dirty. Make sure to remove any oil residues so that the
camera may focus correctly.

4.3 Use of the video camera and the

digital camera
The video camera medicam: Fast, robust, reliable:
The video camera provides constant assistance with the daily routine in a
busy medical practice. Only medicam enables the dermatologist to do a
quick skin cancer screening and to use the expert system Moleanalyzer
as an instant diagnostic aid and a second opinion. Even in a non-stop
operation mode medicam ensures maximum stability and reliability.
We recommend working usually with the video camera.
The digital camera Canon PowerShot: Brilliant, high-resolution,
software-controlled:
The digital camera is an addition to the video camera whenever high-
resolution clinical and microscopic images are needed.
The digital camera is completely controlled by the FotoFinder software
and connected to the system via the CamControl box. The CamControl
box is firmly attached to the camera and also includes the power supply
for the optional dermoscopy lens.
For high-resolution dermatoscopy images with the digital camera the
microscopic lense D-Scope II basic is needed. Images taken with the
digital camera and this lens have a resolution of various million pixels, so
that you can zoom in on the slightest details.

4.4 Using the medicam

FotoFinder offers two medicam models: medicam 500 and medicam
800HD, which can be easily distinguished by the silver or golden camera
case.
Medicam 500’s silver standard enables captures with 720 x 576 pixels,
ratio 4:3. This camera is available with all hardware configurations and
the special lenses Veinscope and FD-Lens.
The gold standard of medicam 800HD enables captures in ratio 4:3 or
16:9 with a resolution of up to 1920x1080 pixels. This camera is only
available in combination with the Silent Medical Server.

11

4.5 The digital camera Canon PowerShot

4.5.1 The digital camera Canon PowerShot

In addition to medicam we recommend using the digital camera
whenever high-resolution digital images are needed, e.g. for
presentations. For high-resolution microscopic images you can
attach the dermoscopy lens D-Scope II basic on the digital camera.
With the digital camera Canon PowerShot you can take high-
resolution images in brilliant quality. Camera and computer are
constantly connected via the CamControl box with integrated control
line. Therefore the entire image capturing process is completely
software-controlled.
You can choose a corresponding profile for each kind of image, e.g.
“Portrait”. In the overview image mode the corresponding profile
needs to be chosen manually; e.g. when using the special lens
Veinscope you can choose between the profiles “Veinscope cross
polarization” and “Veinscope parallel polarization”.
For micro images only the profile “Microscopy” is used. Please select
it, when you want to use the dermoscopy lens attached to the digital
camera.
By clicking on the profile all camera settings are adjusted
automatically. Simply press the release button and you will have
brilliant, standardized images!

Note: The digital camera cannot be used in the screening mode.

Also, microscopic images with the digital camera cannot be
evaluated with Moleanalyzer. Please use the video camera
instead.

6.2 Overview Images

Overview images are necessary to assign individual nevi to particular
body parts. This helps you locate multiple nevi on the patient’s body
when taking the corresponding microscopic images. Individual moles are
marked and numbered in the overview image to clearly define and locate
them.

Guidelines for Overview Images

• Make sure that the room where you set up the system is well
illuminated. Additional light sources may be necessary to avoid
shadows on the patient’s skin.
• For better illumination when using the video camera please
switch on the integrated ring floodlight on medicam.
• Make sure that the patient stands in an upright position and that
all lesions are visible in the overview image (check for hidden
lesions).
• Make sure not to move the camera while taking images. If an
image is not properly focused, undo it and take it again. The
integrated steel spacer on the video camera medicam or a
tripod may help.

6.2.1 Overview Images with the medicam

Take off the dermoscopy lens from the video camera medicam and
select MediCam in the menu bar.

Now click on Overview to start the application.

37
FotoFinder dermoscope dynamic manual

The program switches to the image capturing mode. Please turn on

the ring floodlight on the backside of the video camera.

Freeze the image by pressing the release button on

the camera handle or by clicking on the Freeze button.

If the image is blurred or does not meet your expecta-

tions you can delete the frozen image by clicking the
Liveimage button.

Press the release button again or click on the Save

button if you definitely want to save the image.

The buttons on the back of the medicam can be used for

zooming. Be aware that zoom values for overview images are
not saved in the database and thus are not standardized.

38
dermoscope dynamic manual

Before you can save an image, however, you need to select the
Localization. This is the body region where the lesion is located.
Simply click on the corresponding part of the virtual patient’s body.

You can turn the virtual patient by clicking on the green arrows
above.

When you move the mouse pointer over various body parts they
stand out colored. By clicking on a body section it will be marked in
red and automatically be assigned to the field Localization.

39
FotoFinder dermoscope dynamic manual

To save the image, press the release button of the camera again or
click on Save. After the image has been saved the camera’s live
image will reappear on the screen and you can immediately take the
next overview image(s).

By clicking on Back you can leave this function and

return to the desktop.

You will notice that shooting date and time, treatment and
localization have automatically been added to the patient data
field.

6.2.2 Overview Images with the Digital Camera

Select PowerShot in the menu bar.

40
dermoscope dynamic manual

We recommend connecting the digital camera to the computer

via the USB cable and turning it on before starting the program.

Choose the appropriate camera profile from the list:

Distance piece - to use with FotoFinder spacer
Distance piece close up - to use with the spacer for pictures
with higher zoom level
Microscopy - to use with FotoFinder „D-Scope II“
Portrait freehand - for overview pictures with a higher
zoom factor (e.g. for facial photos)
Snapshot - wide angle for overview pictures
Whole body - to use with tripods for Full Body
Photography, like Studio Stand or
Tower

Use this button to refresh the live image after selecting

a new profile or changing the patient’s position. Be
aware that the preview does not reflect the actual
picture quality.

The further proceeding is identical to point “6.2.1. Overview Images

with the medicam”. Please follow the instructions given there.
The digital camera shoots high resolution images. Especially if
you want to zoom into an overview image later on the excellent
image quality and sharpness of the PowerShot guarantee a
decent enlargement quality.

41

6.2.3 Using the schema

Instead of taking clinical images with the camera, you can also
indicate the location of a nevus on the puppet. Choose Schema
Selection in the drop down menu “Image recording selection”.

Now you can select the body part on the puppet by clicking on it. You
can turn around the puppet with the green arrows.
Save the selected detail directly in database. In the gallery it will
appear as overview image. To mark it, proceed as indicated below
(Marking images).

42
dermoscope dynamic manual

6.2.4 Bodystudio Overview Images

FotoFinder bodystudio is the module for standardized total body
photography. FotoFinder bodystudio together with dermoscope
dynamic offers you the most complete system for the early detection
of skin cancer, also for patients with multiple nevi. The bodystudio
software guides through min. 17 standardized patient poses. In less
than 10 minutes you can obtain a complete documentation of the
skin. These pictures can be used as overview pictures of the
dermatoscopic analysis. Simply mark the nevi in the bodystudio
software with marking arrows. The marked pictures will then appear
automatically in the dermoscope dynamic SmartGallery.

6.2.5 Marking Images (Numbering Nevi)

After you have made all overview images of a patient you can mark
the individual moles on the image. It is essential to mark moles if you
want to take microscopic images of them.

Important: It is not possible to capture microscopic images if

you have not marked them in the overview image before. Each
microscopic image must clearly be assigned to a position in the
overview image.

43
FotoFinder dermoscope dynamic manual

Follow these steps to mark moles (positions) in an image:

Click on the Marker button.

Set the mouse to the desired position in the image and drag a line
away from the mole by clicking the left mouse button. After releasing
the mouse button the line is fixed and a number appears at the end
of the line. The number is assigned automatically by the system.

Continue to mark the next relevant mole (position) in the overview

image. When all moles are marked, press the Marker button again to
switch off the function.

The system numbers all moles (positions) of a patient

consecutively; therefore each number is only assigned once
and micro images cannot be assigned wrongly.

Deleting Positions
Markers can only be deleted if they are not related to any micro
image.
To do so, click on the position in the overview image with the right
mouse button. Select Delete in the following menu.

44
dermoscope dynamic manual

By selecting Line Color you can choose the color of the respective
marking in the context menu.

6.3 Micro Images

For the micro shooting you can also choose at the push of a button
whether to capture the image with the video camera medicam or the
digital camera.

Microscopic images captured with the digital camera are always

about 20X magnified. If you choose the medicam you can select
an enlargement of up to 70X by using the zoom function.

Guidelines for micro images

• Attach the dermoscopy lens to the video camera or the digital
camera and switch on the integrated ring light for micro images.
• Make sure that lens and glass tube are perfectly clean before
you start taking the image.
• Always use 70% alcohol spray (Kodan spray) when taking
micro images. The skin must be completely wet. Dry skin

45
FotoFinder dermoscope dynamic manual

reflects light and can hide the structure of a lesion. Try to avoid
bubbles and hair when taking the image. Shave the spot if
necessary.
• You can use the optional Polarization Kit to work with a
polarization filter instead of immersion liquid.
• Make sure to photograph the lesion in the correct magnification.
If you wish to analyze the image with Moleanalyzer make sure
to take the image with the medicam and 20x magnification.

6.3.1 Micro Images with the medicam

Mount the dermoscopy lens on the medicam and select MediCam
from the menu bar.

Select a marked lesion in the overview image you want to assign a

micro image to.

46
dermoscope dynamic manual

Then click on Micro Shooting.

The program now displays the micro shooting screen. In the left
window you can see the live video image. On the right hand side the
last micro image you have taken of that position is shown (baseline
image), below you can see the overview image of that lesion.

Live image Baseline image Overview image

If a baseline image does not exist yet the correspond-ding

window will be empty.

47
FotoFinder dermoscope dynamic manual

Now switch on the micro lights on the back of the camera, moisten
the skin with alcoholic spray (e.g. Kodan) and place the glass tube of
the dermoscopy lens on the nevus.
While working with the optional polarizing filter, no immersion fluid is
needed. In everything else, follow the instructions normally.
In the menu bar you can choose the desired zoom level. Alternatively
you can adjust the zoom factor with the zoom buttons on the back of
the camera.

If there is already a previous micro image saved (baseline image),

the camera will automatically select the same settings as before.
This guarantees that images are 100% comparable. Nevertheless,
you can still change the enlargement if necessary.

The camera focuses automatically thanks to its built-in auto

focus system.
If you wish to export the nevus to Moleanalyzer later on, make
sure that you use the medicam and that the enlargement is set
to 20X.

Press the release button at the camera handle or click

on the Freeze button to freeze the micro image. The
image will be displayed in original size when it is
frozen.

If the image does not meet your expectations (e.g. if it

is blurred), you can delete the image by clicking on
Liveimage.

48
dermoscope dynamic manual

Press the release button of the camera a second time

or click on the Save button to save the micro image.

After saving the image the software automatically goes on to the next
position. This allows a fast micro shooting session for all moles
(positions) of the patient.
Repeat the last steps to capture more images.

The buttons Previous and Next allow you to jump from one position
to another. When all positions of an overview image have been
called up, the system automatically goes on to the next overview
image in chronological order.

This way you can jump back to the previous image if you want to
make another micro shooting of the same lesion with a different
magnification.
Or select Next if the lesion hasn’t changed since the last examination
and another save isn’t necessary.
If you want to skip a position in further follow-ups (eg. because the
lesion was removed), you can deactivate the corresponding marker.
Simply click the marker with the right mouse button and select
“deactivated”. You can reactivate markers the same way.
You can stop the micro shooting session at any time.
Simply click on Back and you will return to the desktop.

49
FotoFinder dermoscope dynamic manual

6.3.2 Micro Images with the digital Camera

Attach the dermoscopy lens to your digital camera and select the
digital camera PowerShot from the dropdown menu. Then click on
Micro shooting.

Now you have to choose the camera profile “Microscopy” from the
list. To refresh the live preview click

For the next image capturing steps please follow the instructions
described in the previous chapter “6.3.1. Micro Images with the
medicam”.

The digital camera has only one standard magnification for

capturing micro images of about 20X. Therefore, you cannot
select different magnifications and the pictures cannot be
analyzed with the optional Moleanalyzer.

can be started directly from within the FotoFinder dermoscope
dynamic software. It allows you to measure lengths and areas in
captured images with several selections tools. Click this button to
start the measuring program:

Please, keep in mind that a picture is a two dimensional

representation of a 3D body. That’s why it is not possible to
achieve absolute measurements of distances, angles and areas
in the image on-screen. Nevertheless, you can evaluate change
and treatment success comparing measurements.
The menu bar is located on the top of the screen.

Exit the Measure module. If you have made any changes in

the image a safety question will appear before quitting this

8 Special lenses and additional tools

8.1 Tuebinger Moleanalyzer

with medicam
The Tuebinger Moleanalyzer1 allows an exact measurement of
pigmented lesions in order to help the doctor judge if the lesion in
question is benign or malignant. There are two main purposes of this
image analysis program:
• The image analyzer was programmed to detect changes in
pigmented lesions during the follow-up examination of patients. With
this program, it is possible to detect minimal changes in the size of
the lesion, its pigmentation, its structure and border. This makes the
indications for excision of moles more secure.
• Specific scores for each individual parameter give indications of
malignancy. The scores vary between one and eight. Higher scores
than 6 may indicate an increased probability of malignancy.
However, there is not yet a clear cutoff-point for determining
malignancy. The knowledge of an experienced physician is needed
to finally interpret these images.

Moleanalyzer is not an automatic diagnosis system!

After the analysis, a combined score will indicate the risk of malignancy
based on the calculated parameters. For better patient communication
you might want to show this result in form of a bar with a continuous color
spectrum between red, yellow and white. This function helps you classify
the nevus, by indicating white = without pathological findings, yellow =
needs to be controlled, red = dangerous.
In order to improve comparability, Tuebinger Moleanalyzer makes it
possible to analyze two images at the same time on the same screen.

1
Note: Moleanalyzer is known as Dynamole in the USA.

62
dermoscope dynamic manual

Starting Moleanalyzer
1. Select the micro image you want to analyze in the SmartGallery.
2. Start Moleanalyzer directly by clicking on the corresponding button:

3. If you wish to analyze two images at the same time, start the image
compare function for microscopic images and select the required
images by using the scroll bar. Then click on the button
Moleanalyzer. Both images will now be analyzed on the same
screen. If an overview image is shown aside of the microscopic
image only the microscopic image will be analyzed.
4. The analyzer starts automatically and will indicate you parameters
and score when finished.
You will then see the following screen:

63
FotoFinder dermoscope dynamic manual

The border of the lesion is indicated with a blue line. The black lines mark
structures within the lesion. Afterwards, the program determines size,
geometric values and color values.

Important: The score can only be calculated, if:

1. the nevus in question is melanocytic and
2. the image has been taken in 20X enlargement.
3. the image has been taken with the video camera medicam
500.
Make sure to have chosen the right enlargement already when
taking the micro image, so that a later analysis of the lesion is
possible. Please note that not every lesion is suitable for an
analysis with Moleanalyzer! Make sure to use the video camera
if you wish to analyze the lesion with Moleanalyzer later on.

Additional functions of Moleanalyzer

Change border
If lesions are very bright or irregular the border may not be found. In this
case, you can enlarge or reduce the border by using the function
“Change border “.

Print results
The results of the analysis can be printed directly in A4 format by clicking
the button “Print”.

8.2 Trichoscan
The expert system Trichoscan is used for measuring parameters of hair
growth and loss. The module automatically determines the number of
hair in a certain area and its anagen-telogen rate.
Please read the Trichoscan manual for the preparation of the analysis.

64
dermoscope dynamic manual

After having executed all preliminary steps (shorten and dye the hairs),
please take a microscopic image of the scalp in 20X enlargement. To do
so, proceed as described in the chapter “Capturing micro images”. After
you have taken the micro image, go back to the desktop and select this
image in the SmartGallery.
You can now start the Trichoscan software by clicking on the
corresponding button.

The meaning of the individual measurement parameters is described in

the Trichoscan manual.

8.3 Bodyscan pro

The expert system Bodyscan pro helps you to detect new and changed
lesions automatically. Complete skin cancer prevention must include the
control if new moles have appeared since the last examination.
Bodyscan pro compares the clinical photos of the last examination with
the recent ones. Every new lesion is marked explicitly and magnified in

65

8.4 FotoFinder special lenses

8.4.1 Veinscope
(optional for both cameras)
This lens offers excellent image quality for macro images. Thanks to an
integrated continuously adaptable filter settings, can be adjusted
between cross and parallel polarization. Fields of application:
• Brilliant documentation of acne, spider veins, couperosis and
redness without any reflections with cross polarization.
• Plastic visualisation of the skin surface with parallel
polarization. Thus unevenness and wrinkles are perfectly
shown.
• Documentation of large moles with a diameter of up to 7cm.

Veinscope for the medicam

Veinscope is an optional lens which can be attached to the video
camera via a bayonet ring. Veinscope has 30 built-in light diodes; its
metal distance holder avoids impact of room-light on the image. This lens
offers 10Xmagnifications and the lens coverage has a maximum
diameter of 72mm.

8.4.2 Special lens for fluorescence diagnosis

(for medicam 500)
FotoFinder FD-lens for Fluorescence Diagnosis delineates actinic
keratosis and basal cell carcinomas from the surrounding tissue.
Therefore, a photo sensitizer is applied to the patient’s skin. After an
incubation time of 3 to 5 hours porphyrin is induced in tumor cells. Under
the special blue light of the FD-lens the porphyrins get excited to
fluoresce. The tumor becomes visible and can be delineated from the
healthy skin.
Thanks to the lens’s chrome spacer and the specially designed cloth the
skin is completely covered when the image is taken. You can change
between white and blue light on the camera’s backside.
