We live in a neighborhood with lots of dogs. During the pandemic and since then, many dog owners who work from hme meet up twice a day during the week to gather on the lawn
Understandingly, not everyone is comfortable with dogs running around freely; ideally we would have a proper dog park.
We decided to petition the city for the dog park but we wanted hard data to share.
Make a survey that would be short and could let us account for double entries (aka poison submissions) which might skew the results.
We tried going door-to-door to conduct the survey in order to prevent double counting; however, we found few people would answer the door.
We decided to create a survey that people could fill on their own time. We left a flyer with a unique QR code at the door.
I didn't have much time build a whole web-app. Instead, I created a simple form using Typeform which unlike google forms allowed me to include a hidden field value for tracking.
This script would generate the survey flyers, and then my neighbors and I distributed them around the neighborhood marking down which QR code was left at each address.
If someone submitted the survey frm the same QR code we could see it. We could also see who had not submitted the survey response.
