Subject: Application for Medical Leave

Respected Sir/Madam,

I hope this application finds you well. I am writing to formally inform you about
my medical condition, due to which I was unable to attend university for a
significant period. I suffered an elbow fracture on February 23, 2025, and was
admitted to AIIMS for surgery and treatment from February 27, 2025, to March
5, 2025. As per my discharge summary, my doctor has advised me to take 28
days of rest starting from March 5, 2025, to ensure proper recovery.
So I am seeking medical leave from 27th February to 28th March, 2025.

Due to my medical condition and hospitalization, I was unable to attend my MST


examinations during this period which was on 24th March, 25th March and 27th
March, 2025. I kindly request you to consider my situation and grant me the
necessary leave. I have attached my medical records as proof of my condition.
Please let me know if any further documentation is required. I would be grateful
for your support in this matter.
