I​ am writing to formally request a medical leave of absence for four (4) days, effective from
05/11/2025 to 08/11/25, inclusive.

I​ have been advised by a medical practitioner to take complete rest and recover from a
sudden illness severe viral infection, high fever. I plan to rejoin school on 10/1125

I​ understand the importance of following school protocols for safety and tracking, and I
deeply apologize for this oversight. I assure you that this departure was solely a
consequence of my immediate physical distress, and I accept full responsibility for not
adhering to the communication process.
