Subject: Application for One Month Medical Leave and Request for Transfer Consideration

Respected Sir,
I am Muhammad Farooq, serving as a Chemistry Teacher for the past nine years at the Centre of
Excellence Boys, Pir Mahal. During my tenure, I have been honoured multiple times as the Best Teacher
and Best Result-Oriented Teacher in recognition of my dedication, hard work, and consistent contribution
to the academic excellence of my students.
Sir, I would like to express my deep concern regarding the recent incident involving the Principal, which
has caused me considerable mental distress. Despite submitting my medical leave application and
assuring the Principal that I would soon provide the medical certificate, my request was unfortunately not
accepted. The situation and subsequent threats have adversely affected my mental and physical health,
and I am currently suffering from severe back pain.
I state with utmost sincerity that I have always performed my teaching duties with complete honesty,
dedication, and a strong sense of professional responsibility. In light of my current health condition, I
humbly request one month’s medical leave from 15th October 2025 to 15th November 2025.

Moreover, I would also like to draw your kind attention to the recent advertisement for the post of
General Science Teacher at Daanish School Chishtian. Being married, with five dependent family
members including my mother who requires my care, and as my daughter is also studying at Daanish
School Chishtian, it would be a great support for my family if my transfer from Centre of Excellence
Boys, Pir Mahal to Daanish School Chishtian could kindly be considered.
I sincerely hope that you will take into account my years of dedicated service, professional integrity, and
present circumstances, and will graciously approve my leave and transfer request.
